Understanding Low Testosterone (Low “T”)

Testosterone is a hormone fundamental to male development and health. From contributing to muscle growth and voice deepening during puberty, to maintaining muscle and bone strength in adulthood, testosterone plays a significant role. Furthermore, it is integral to male sex drive.

After the age of 30, testosterone levels in men typically begin to decrease gradually. This decline can sometimes be accompanied by a reduction in sex drive, leading many to erroneously link their lowered interest in sex to aging. However, a myriad of symptoms related to low testosterone can also be associated with other medical conditions such as diabetes, depression, high blood pressure, and coronary artery disease.

Normal total testosterone ranges from approximately 300 to 800 nanograms per deciliter (ng/dL), varying by lab. A score below normal could be due to several conditions, including testicular injuries, hormonal disorders, chronic liver or kidney disease, obesity, and more. Physicians aim to exclude these potential explanations for symptoms before attributing them to low testosterone levels.

In terms of treatment, a decline in testosterone level with age is expected, but treatment is sometimes considered for symptomatic individuals. This could include hormone injections, testosterone replacement, or topical gels. Costs can range from $50-$400 depending on insurance. However, the first step is to get tested.

Erectile Dysfunction (ED) and Testosterone

Low testosterone levels may affect your sex life, including a decrease in sex drive or the development of erectile dysfunction (ED). ED is characterized by difficulty achieving or maintaining an erection suitable for sex.

ED is multifactorial. It can be related to chronic conditions that reduce blood flow to the penis, like high blood pressure, diabetes, and heart disease. Psychological issues like depression, anxiety, relationship problems, and neurological conditions can also contribute to ED.

While testosterone replacement therapy (TRT) can improve sex drive in men with low testosterone, it may not be sufficient to overcome ED. In such cases, PDE-5 inhibitors like Viagra, Levitra, Cialis, or Stendra, which enhance erections by increasing blood flow to the penis, may be required. Some men with low T do not respond to these drugs without TRT. However, the efficacy of TRT on improving erections may not be long term.

Benign Prostate Hyperplasia (BPH)

As men age, the prostate naturally enlarges. This can lead to a condition called benign prostatic hyperplasia (BPH), which causes the urethra to be compressed. This may weaken the urine stream, cause frequent urination, and prompt you to wake up frequently at night to urinate.

BPH is common in older men, and is not related to prostate cancer. Its symptoms, which can vary in severity, include a hard time getting started with urination, dribbling at the end of urination, and feeling like you still have to pee even after you just went.

Diagnosis and treatment of BPH are individualized, based on age, overall health, the size of the prostate, and the impact of the condition. Lifestyle changes, such as pelvic floor exercises, fluid intake regulation, and reduced caffeine and alcohol consumption, can help manage mild to moderate BPH. For some, medication may be necessary to relax the muscles in the prostate and bladder, or to shrink the prostate.
